This happens if you somehow got two clues at once and completed the other, or if you somehow solved a clue without having it in your inventory. Since neither should be possible anymore, it must be some new variation that caused that. Is this ringing any bells, did you do anything similar that might have caused this?

I'll test if dying with the clue would cause it to bug. So far that hasn't really been the case.
