Fix Hotspots

Hot spots are a great idea but some things about them are stupid.

1. They need to be in the area never outside it, there are spots like at red dragons where you can hold it without being in the area.

2. If someone leaves the HS they should be skulled, you go there to attack someone for the HS risk full armor they step out and now they have no skull, you don't even know if they are skulled or not when you attack due to the red skull. They should be skulled for same length of time as if they attacked when leaving HS.

3. Maybe even no logging inside the hot spot area, it should be dangerous and they aren't huge to move out of to logg.

Also make it so lvl 3 or below lvl 20s or something can't hold hotspots. So many trolls...
